Jersey City Medical Center created Green Apple Rx, JCMC Food FARMacy to serve community members living in Hudson County who are food insecure and have a chronic disease such as diabetes, obesity or hypertension. Program participants will be eligible to receive free nutritional counseling by a Registered Dietitian and free food through our partnership with ACME Supermarkets, GOYA, the Community Food Bank of NJ and other local community agencies and donors. The program is free, but registration and participation with our Greenville Primary Care Team is required.
